About Bridge to Home Housing
What We Do:
We provide Housing Stabilization Services, a Medical Assistance (MA) benefit that helps individuals with disabilities and seniors find and maintain housing. Our services are available through both the State Plan and Home and Community-Based Services (HCBS) Waiver programs.
At Bridge to Home Housing, our mission is to empower individuals with disabilities and seniors by providing compassionate, person-centered housing stabilization services. We are dedicated to supporting our clients in finding, securing, and maintaining safe, stable housing, fostering independence, and enhancing quality of life across Hennepin County.

Housing Stabilization Consultation:
Person-centered planning for those without MA case management.
Housing Sustaining:
Ongoing support to maintain stable housing.
Housing Transition:
Helping individuals find and secure stable housing by assisting with rental applications, lease agreements, landlord communication, and access to financial resources.
Moving Expenses (up to $3,000):
Assistance for eligible individuals transitioning from institutional or provider-operated settings.
Eligibility:
● 18+ on MA with a documented disability or disabling condition
● Assistance needed with communication, mobility, decision-making, or behavior management
● Experiencing housing instability or at risk of homelessness/institutionalization
