About Bridge to Home Housing

What We Do:

We provide Housing Stabilization Services, a Medical Assistance (MA) benefit that helps people with disabilities and seniors find and maintain housing. Our services are state plan (not waiver) Home and Community-Based Services (HCBS).

At Bridge to Home Housing, our mission is to empower individuals with disabilities and seniors by providing compassionate, person-centered housing stabilization services. We are dedicated to supporting our clients in finding, securing, and maintaining safe, stable housing, fostering independence, and enhancing quality of life across Hennepin County.
