We provide Housing Stabilization Services, a Medical Assistance (MA) benefit that helps people with disabilities and seniors find and maintain housing. Our services are state plan (not waiver) Home and Community-Based Services (HCBS).

Eligibility

  • 18+ on Medical Assistance (MA)

  • Documented disability, including physical, developmental, mental health, or substance use conditions

  • Need help with communication, mobility, decision-making, or behavior management

  • Experiencing housing instability (homeless, transitioning from an institution, or at risk of homelessness)

  • 18+ on MA with a documented disability or disabling condition

  • Assistance needed with communication, mobility, decision-making, or behavior management

  • Experiencing housing instability or at risk of homelessness/institutionalization
